• One (1) year validity from the date of Summit (June 1, 2018 – June 1, 2019).

• Auction form due to Denee Conner at dconner@teamcatalyst.com by April 20, 2018.

• Auction items and gift certificates due to Catalyst Marketing c/o Denee Conner: 1466 Van Ness, Fresno, CA 93721 by May 1, 2018.

• If applicable, please provide materials such as location, marketing pieces, and/or brochures with items.
